If a child is inscribed in the passport of the parents - this is indicated in the application, and the visa is issued for free on the passport of the one from parents in whose passport the child is inscribed. The photo and page in the passport with a note on the inscribed child is submitted for the visa along with the parents' documents.

If a minor child (under 18 years of age) has different surnames with the parents, the Immigration Service necessarily requires the addition of a copy of the child's birth certificate and translation into English (notarization not required).
