Visa application
Required documents:
- passport (validity period - not less than three months from the end of the trip);
- a photocopy of the passport (page from the photo);
- a photocopy of the pages of the used passport if it had Schengen or Polish visas;
- The application form is completed online and registered on the website of the Ministry of Foreign Affairs of Poland. The application will be assigned a unique number, information about the applicant will be contained in the bar code. The questionnaires in printed form are not accepted. The questionnaire should be printed after filling in (all 4 pages) and signed;
- one color photo of 3, 5x4, 5 cm (not "older" than 6 months), which must be glued to the questionnaire;
- document confirming the purpose of stay (voucher or confirmation of payment for the tour);
- a certificate from the place of employment, on the form with the requisites of the organization, indicating the position, length of service and salary;
- confirmation of the availability of funds: an extract from the bank account, traveler's checks or a copy of the credit card (with a certificate from the bank on the availability of funds);
- a copy of electronic air or railway tickets round-the-clock or ticket confirmation;
- confirmation of booking and 50% payment for the whole stay in the country;
- the original and a copy of the medical insurance valid throughout the Schengen area and for the entire duration of the trip. The amount of insurance coverage must be at least 30 000 EUR.
Private entrepreneurs must provide a copy of the certificate of registration of an individual or legal entity, as well as a certificate of registration with the tax authority (TIN).
For children under 18 years of age: a photocopy of the birth certificate (if the child has his passport, then also two photos of 3, 5x4, 5 cm). If a minor traveler travels only with one of the parents or an accompanying person, a photocopy of the notarized application (power of attorney) from the second (both) parents is required, indicating the purpose and terms of the trip.
Residents of St. Petersburg, as well as Leningrad, Murmansk, Pskov, Arkhangelsk, Novgorod, Vologda regions, Republics of Karelia and Komi can apply for a visa to the Consulate General of Poland in St. Petersburg.
Residents of the Irkutsk and Chita regions, the republics of Buryatia and Tyva, the Krasnoyarsk Territory, the Agir-Buryat and Ust-Ordyn Buryat Far Eastern regions of the Russian Federation should apply for a visa to the Consulate General of Poland in Irkutsk.
Residents of the Kaliningrad region can get a visa at the Consulate General of Poland in Kaliningrad.
The visa processing deadline is 5-10 working days, the consular fee is 35 EUR. An urgent visa is issued for 3 working days, the consular fee is 70 EUR. When submitting documents via the Visa Center, an additional service fee of 17 EUR is charged. It is paid in rubles at the exchange rate of the Central Bank on the day of payment.
